Brief Specification Description: Provide basic public health services such as occupational health surveillance, health management, chronic disease prevention and control, infectious disease prevention and control, health education, women' healthcare and family planning, and vaccination to enterprises within the bonded area, as well as basic medical services such as outpatient services and laboratory tests for enterprises and employees in the area, to meet daily medical needs of the park's enterprises and employees, and to ensure a favorable development environment for the area to attract, retain, stabilize, and please businesses

2. Qualification Requirements for Suppliers
(a)满足《中华人民共和国政府采购法》第二十二条规定；
(a)Meet the provisions of Article 22 of the 'Government Procurement Law of the People's Republic of China';
(b)落实政府采购政策需满足的资格要求：
（1）本项目专门面向中小型企业采购，执行《的通知》（财库〔2020〕46号）、《财政部关于进一步加大政府采购支持中小企业力度的通知》财库(2022] 19号。
（2）扶持监狱企业、残疾人福利性单位，并将其视同小微型企业
(b)Qualification requirements to be met to implement government procurement policies: (1) This project is specifically oriented towards procurement by small and medium-sized enterprises, implementing the 'Notice on the Administrative Measures for Government Procurement to Promote the Development of Small and Medium-sized Enterprises' (Caiku [2020] No. 46) and the 'Notice of the Ministry of Finance on Further Increasing Support for Small and Medium-sized Enterprises in Government Procurement' Caiku [2022] No. 19. (2) Support prison enterprises and welfare units for persons with disabilities, and treat them as small and micro enterprises.
(c)本项目的特定资格要求：
（1）供应商需具有卫生行政部门核发的《医疗机构执业许可证》。
（2）未被“信用中国”（www.creditchina.gov.cn）、中国政府采购网（www.ccgp.gov.cn）列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单。
（3）单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项目下的政府采购活动。
（4）本项目专门面向中小型企业。
（5）本项目不接受联合体形式投标。
(c)Specific qualification requirements for this program: (1) The supplier must possess a 'Medical Institution Practice License' issued by the health administrative department. (2) The supplier must not be listed the list of dishonest persons subject to enforcement by 'Credit China' (www.creditchina.gov.cn) and the China Government Procurement Network (www.ccgp.govcn), nor on the list of parties to major tax violation cases or the list of records of serious illegal and dishonest behavior in government procurement. (3) Different suppliers whose persons in are the same, or who have direct holding or management relationships, shall not participate in government procurement activities under the same contract project. (4) This project is specifically oriented towards and medium-sized enterprises. (5) This project does not accept bids in the form of a consortium.

1.根据上海市财政局规定，本项目招投标相关活动在上海市政府采购云平台（网址：http://www.zfcg.sh.gov.cn）电子招投标系统进行。投标人应根据《上海市电子政府采购管理暂行办法》等有关规定和要求执行。投标人在政府采购云平台的有关操作方法可以参照政府采购云平台中的专栏有关内容和操作要求办理。 投标人应在投标截止时间前尽早加密上传投标文件，电话通知项目负责人进行签收，并及时查看政府采购云平台上的签收情况，打印签收回执，以免因临近投标截止时间上传造成无法在开标前完成签收的情形。未签收的投标文件视为投标未完成。
2、供应商的投标响应活动应当符合有关文件规定、符合上海市政府采购云平台的设置要求，如因供应商自身原因导致投标失败，由供应商自行承担相应责任与损失，采购人及招标代理机构不对此负责。
